In this video, I am going to show you how to complete and submit the E-Rate Form 470, the first form in the E-Rate process. This is a new version of the form, just updated and released in December 2013. To begin, go to the USAC School and Library's webpage. The URL is usac.org/sl. In the menu on the left-hand side of the page, under resources and tools, choose the first option, "Apply Online." This will bring you to the page where you can apply and submit all of the E-Rate forms online. A couple of things to be aware of before you get started: 1. Internet Explorer is the only browser that you can use to submit your forms online. Other browsers have not been tested and may cause problems and errors. 2. Allow pop-ups in your browser. Turn off your pop-up blocker. There will be warnings, instructional guides, and help that will pop up within the E-Rate forms. If you don't allow pop-ups, you may miss important information. To get started with the Form 470, click on the blue button on the left that says, "Create Form 470." The first thing you need to do is look up your billing entity. You can do this either by zip code or by entity number. If you're not sure what your entity number is, you can do a zip code search to bring up a list of all the billing entities in your area. Choose the one that is yours. If you do have your billing entity number, you can enter that directly. To find out what your billing entity number is, you can look at a previous E-Rate form.
