So, if you're somebody who's filling out this form, there are instructions as to when to use it. So, there's a DV165 that has already been issued by the court, and you want to file a document or form that includes confidential information. So, how would you use this cover sheet? You need to make two copies of the documents that you do want to file and complete this form. Place this form on top of each of the copies that you want to file and then file them with the court. The clerk has to review and approve the redacted version of the documents attached to this cover sheet before actually filing. If the court doesn't do it, the clerk must do it. So, the judge takes it in their chambers and reviews and approves the redacted version. If it is approved, then the court will file the redacted version in a public file, and then file the unredacted version and this cover sheet in a confidential file. To complete your name and the other party to this case and whether this DV165 order was made in this case or another case. If it was another case, then include the case number and the county in which it was filed. Then, include the minor party's names that were protected by the confidentiality order. Go to attachment two and type in "DV165 attachment two", then the name of the minor children. Attach two copies of the following documents.
